JAKARTA, Jan. 28 (Xinhua) -- Human error remained the dominant cause of transportation accidents in Indonesia last year, according to Soerjanto Tjahjono, head of the National Transportation Safety Committee (KNKT), on Wednesday.

"In many cases, the human factor is still the biggest contributor, whether related to fatigue, negligence, or non-compliance with safety procedures," Soerjanto said at the 2025 KNKT Performance Achievement Media Release event held in Jakarta.

He noted that the committee's findings showed a consistent pattern across different modes of transportation, including road, maritime, and aviation sectors.

Although each mode of transportation carries distinct risk characteristics, Soerjanto said, the underlying causes of accidents tend to follow a similar pattern, with human-related factors playing a central role.

To prevent the recurrence of similar incidents, the KNKT has issued at least 1,481 cross-modal safety recommendations between 2015 and 2025, most of which focus on strengthening control mechanisms and supervision, he said.

Soerjanto urged all transportation stakeholders, including operators, regulators, and local governments, to fully implement the recommendations as part of broader efforts to improve safety.

"The KNKT's investigations are oriented toward learning and prevention, with the ultimate goal of ensuring a safer national transportation system," he added.
